Healthy New York Offer of Coverage by Business within the Last 12 Months
Healthy NY is a program designed to make reduced-cost, comprehensive health insurance available to small employers. One of the conditions to be eligible for this program is that the business must not have provided comprehensive insurance coverage to its employees in the past 12 months. In keeping with this requirement, a business that has offered coverage to its employees within the last 12 months will not be eligible for Healthy NY, even when the ownership, EIN, name or similar type of change occurs, as long as the same employees are being offered coverage. This applies even if the business is adding new employees.
New Group Application and Detailed Instructions
A new version of the Univera New Group Application for Commercial Health and Dental Products is now available. We value our relationship with you, so we made every effort to redesign the form to make it easier for you to complete.
Here are some of the important changes that we made:
- Added detailed instructions to explain how to complete most fields, with examples where appropriate
- Removed the calculation of participation and Full Time Equivalent for medical products
- Streamlined documentation requirements. Tax returns are required in fewer situations. See instructions for complete details.
You may begin using this form immediately for submissions of both large and small groups.
Effective January 1, 2019, we will only be accepting this new form.
